Clinic Registration Form - SignUp Page - should have doctor and admin login only. Signup page should have Login to Our Website Doctor and Admin login and Book Now for Consultation. Click on Book Now for Consultation it should ask to fill Patient Details then click next, it will list all doctors in the clinic along with their departments. select the doctor and click on Take Appointment. Once you click Take Appointment it should display Doctor profile with charges and department. click available slots. Once you click for Available slots it should take you to Appointment Taken Page. Then select the free slot available and click on Send Request button. Doctor logs in to his website and select the Pending Appointment. we should give option to Accept or Re-schedule his appointment. Once reschedule, the SMS must be triggered to the patient mobile as he must be notified and send email on the same. Once the patient accepts it, it will go to the doctors pending appointment again. Over there he can accept it and diagnose the patient. Then in Patient Diagnosis Information Form, it should have Date, Name, Mobile Number, Age, Complaints, Diagnosis, Investigation, Treatment, Address. Then in History of Treated Patients we can have all the Patient Information for the particular doctor and we can search by weekly, bi-weekly, annually. Admin Role -> Clinic stats which includes weekly appointments - how many patients visited last week compared to the current week. How many patients came for a particular month and How many patients came for a particular year. Income of clinic. Total Number of Registered Patients and view the Patient Details by clicking on hyperlink button. Total Number of Doctors and view the Doctors by clicking on hyperlink button. View Other Staffs. Add/ Remove Doctors, Patients and Other Staffs. Then in History of Treated Patients we can have all the Patient Information for all the doctors and we can search by weekly, bi-weekly, annually. Reports - financial health income clinic monthly/ quarterly/ annually Expense summary reports.
